Route one: what Vapofil borrows from the prostate literature
Saw palmetto and nettle are the two names here with substantial clinical research, and almost all of it concerns lower urinary tract symptoms rather than libido. That is a different outcome from the one the bottle is sold for, and this page does not convert one into the other.
The proposed mechanism for saw palmetto is inhibition of 5-alpha-reductase, the enzyme that converts testosterone to dihydrotestosterone. It is the same enzyme finasteride and dutasteride act on, which is why the rapid evidence review of benign prostatic hyperplasia is a useful piece of background before adding a botanical on top of one of those drugs. Nettle root’s proposed mechanism is different again and, by the review's own account, still unclear: sex hormone binding globulin, aromatase and prostate steroid membrane receptors are the candidates, with 5-alpha-reductase considered less likely.
What that means practically is this. If you are taking the capsule hoping for the urinary outcome, the doses on this panel are a twentieth and a twelfth of the studied ones, and the studied ones largely did not beat placebo. If you are taking it for the reasons the bottle is sold for, this route is background rather than mechanism.
Route two: the circulatory idea behind Vapofil

Icariin, the compound horny goat weed is standardised for, inhibits phosphodiesterase-5 in cell culture. That is the same enzyme family the prescription drugs in this area act on, which is why the ingredient appears in almost every product on this shelf.
The distance between that sentence and a clinical effect is large and worth stating. The urology review is clear that the published work is preclinical: cell culture and rat models, with no human trial of icariin for this use. A compound that does something in a dish at a concentration a laboratory chooses is not the same as a compound that does something in a person at 8 mg of a 10:1 extract.
This website therefore describes the circulatory route as a rationale rather than as an outcome, and the horny goat weed line matters more for the safety page than for this one: it carries interaction literature with anticoagulants and with blood-pressure medication.
Route three: what Vapofil does and does not say about hormones
Two names carry testosterone literature. The 2022 meta-analysis of tongkat ali pooled five randomised trials and reported a significant increase in total testosterone at the doses those trials used, which were 200 mg a day and upward of standardised extract. The boron depletion and repletion experiments supplemented 3 mg of boron a day against a boron-poor diet and reported raised 17-beta-estradiol and testosterone.
This panel carries 10 mg of tongkat ali extract and one milligram of elemental boron. Neither is the studied amount, and nothing on this website says Vapofil raises testosterone. The 2024 systematic review of testosterone boosters examined 52 studies across 27 marketed ingredients and found that most fail to raise total testosterone at all; tongkat ali was among the handful it rated possibly effective, at the doses those trials used.
What Vapofil cannot do, stated plainly
- It is not a treatment for erectile dysfunction, and it is not a substitute for being examined. Persistent difficulty is associated with cardiovascular disease closely enough that an umbrella review of the systematic reviews treats it as a marker worth acting on.
- It does not work on demand. Nothing on this panel has an acute effect and the seller does not claim one.
- It does not replace the things that move this outcome most. The meta-analysis of lifestyle factors puts physical activity, alcohol, smoking and sleep at the front of that list.
